It is common for patients from Jacksonville, Ponte Vedra to come to see us on consultation for liposuction, when in fact the Tummy Tuck (Abdominoplasty) is the procedure they need, because they have excess skin and fat, poor skin elasticity and weakened abdominal wall muscles. Sometimes the reverse happens, we see patients for  a tummy tuck, when they just need liposuction to remove the excess localized abdominal fat. The abdomen is an area where we often accumulate excess fat. The weight of this excess fat will gradually stretch the skin and can cause stretch marks and looseness of the skin that can hang over the pubic area. Pregnancies, excess intra abdominal fat and aging can also accelerate this process, resulting in additional stretching, weakness and separation of the abdominal muscles specially the two rectus muscles (the muscles used for “sit ups”).

Tummy Tuck (Abdominoplasty) is the procedure of choice when there is excess skin and fat and poor skin elasticity with stretch marks, hanging excess skin and fat, bulging of the abdomen due to excess fat and/or weakened or separated muscles. Diet and exercise cannot remove or tighten loose or hanging skin. A tummy tuck is a procedure where the loose, stretched out skin and fat of the lower abdomen from the belly button (umbilicus) down to the pubic area is removed, the stretched out belly button is rejuvenated and if needed the weakened or separated abdominal wall muscles are tightened. The skin above the belly button is lifted up to the lower ribs to tighten the muscles and to pull down the skin. Liposuction in that skin and fat that has been lifted during tummy tuck can result in complication and more risks of healing problems. The scar for tummy tuck is just above the pubic area, but extends from hipbone to hipbone, below the bikini line and there is a minimal scar around and inside the umbilicus. The scar can be well covered under bathing suite. Recovery period for tummy tuck is about two to three weeks.

Liposuction is the procedure of choice when there is localized excess fat on the abdomen and there is good skin elasticity and no hanging skin or weakened and separated muscles. That excess fat just does not go away with diet and exercise. Liposuction is a procedure where a narrow tube called cannula is inserted through a tiny incision to suction the excess fat. By pushing the cannula through the fat layer, the fat cells will break up, and suctioned out and excess fat is removed. It does not tighten the skin. The scar for liposuction is minimal and recovery period is two to three days. Recently different types of laser liposuction are being offered to tighten the skin and there is evidence that laser can tighten the skin a little bit, but not enough to replace tummy tuck.

How about those patients that have excess fat on the abdomen and also limited looseness of skin just above the pubic area or on the lower abdomen? The procedure of choice is a mini tummy tuck where a small amount of skin is removed above the pubic area without extensive undermining of the skin and without alteration of the umbilicus, combined with full liposuction of the abdomen. Since the skin is not lifted liposuction can be done safely without risk of delay healing.
