Liposuction is the removal of unwanted resistant excess fat deposit, fat lumps, and bulges. Resistant fat deposit can be on the flanks, abdomen, hips, inner, outer thighs, arms, knees and back. This problem may be inherited. Liposuction is often used for gynecomastia, male breast reduction and for treatment of double chins.

Dr. Nezami's goal is to obtain the best possible body contouring and sculpting with a smooth and natural look and most pleasing aesthetic result, and not just maximize the amount of fat removed. A well performed liposuction should not have any dimpling or indentations or any irregularities in the skin and should look like the fat bulge never existed.

Once those excess fat cells are removed, the removal is permanent. However if the person does not follow a proper diet or exercise the remaining fat cells could enlarge. Younger patients will see a better result of their liposuction, mainly because of the elasticity of their skin.

Preparing for your Liposuction Surgery

We advise our patients in Jacksonville and Jacksonville Beach to avoid taking certain medications such as aspirin and Ibuprofen, vitamin E and any other medication that can affect the blood clotting system for two weeks before and after surgery (If you are taking any blood thinner, do not stop until you get authorization from the doctor who prescribed it). This is required to prevent any bleeding complications. If you are a smoker, stop smoking at least two weeks before and after the surgery. Smoking interferes with blood flow to the skin and prevents the rapid healing and may cause delayed wound healing or even tissue loss. Dr. Nezami advises his patients, two weeks prior to their liposuction surgery, not to take any herbal and diet medications without his approval.

A small incision is made near the area where fat is being removed. Dr Nezami makes one or two small incisions in a hidden area for each area suctioned.

Liposuction Procedure

Liposuction is a surgical procedure used for localized fat reduction. There are three techniques used for liposuction; traditional liposuction, tumescent liposuction and ultrasound assisted liposuction. We advise our patient in Jacksonville and Ponte Vedra Beach area to let Dr. Nezami make the final decision on the choice of the liposuction technique that he thinks is best for them.

Traditional Liposuction

In traditional liposuction the plastic surgeon will mark all the areas that liposuction is needed to remove excess fat. Then in the operating room under general anesthesia the areas to be liposuction are injected with moderate amount of a solution that contains salted water, local anesthetic and adrenaline to contract the blood vessels and also anesthetize the area. He will make a small incision near the area to insert the cannula (suction tube). Under suction the cannula is then moved back and forth in an organized fashion, first suctioning the highest point then gradually to the lowest and each time about one or two drop of fat is suctioned out.

Tumescent Liposuction

A very large amount of the tumescent fluid is injected by the plastic surgeon into the fatty tissue under the skin. The tumescent liquid contains salt water, local anesthesia and adrenaline. Through a small opening a cannula is inserted into the fatty tissue. The plastic surgeon vacuums out the fat. With tumescent liposuction, the plastic surgeon must infuse a very large volume of tumescent fluid (two to six times as much tumescent solution as the volume of the fat to be removed). Because of the large quantity of solution injected, it will be harder to precisely judge the evenness of fat extracted or remaining due to the swelling from the large amount of fluid injected. In our Jacksonville Cosmetic Surgery Center, Dr. Nezami prefers to use a combination of traditional and tumescent technique, precisely for this reason. Dr. Nezami is known to be a perfectionist. With the tumescent technique it is harder than the traditional technique to achieve the same evenness of fat extraction. Tumescent liposuction can usually be done under sedation.

Ultrasonic Liposuction and Laser Liposuction

Ultrasonic liposuction uses sound waves or ultrasonic energy to break down fat cells Laser liposuction uses the laser light to melt the fat cells. Once the fat cells are broken and transformed into liquid form, then the plastic surgeon will remove the liquefied fat with cannula. This procedure is less strenuous on the plastic surgeon. Because the tip of the ultrasound or laser probe become very hot and can burn the tissue it must be done with large amount of tumescent fluid to cool down the probe. As explained above due to the large amount of tumescent fluid injected it is more difficult to judge the amount of fat removed or remaining. We often see patients that too much fat was removed during this type of liposuction or the area liposuctioned is very uneven and lumpy.

Duration of Liposuction Surgery

Depending on the size of the areas and the amount of fat being removed, liposuction surgery may take two to three hours.

Scarring of Liposuction Surgery

The scarring in liposuction procedure is minimal. The incision for liposuction is less than quarter of an inch.

Recovery period after Liposuction Surgery

It will take several weeks to have a complete recovery. Recovery after liposuction surgery varies from one person to another, it might take between two to four weeks. We advise our patients in Jacksonville and Jacksonville Beach to start walking around as soon as possible to reduce swelling and prevent blood clots. Most of the swelling will be gone between six to 8 weeks .To minimize the swelling and to get better and smoother result we ask our patients to wear compression garment over the suctioned area. You need to drink plenty of water the first few days after your surgery.

Candidates for Liposuction Surgery

If you are considering liposuction surgery, you must be at least 18 years old, in good health, and not pregnant or be nursing. There is no age limit to have liposuction surgery. Again liposuction is not a weight loss procedure. We advise our patients in Jacksonville and Jacksonville Beach to try to reach their ideal weight first and then use liposuction to correct localized resistant fat deposit on their flanks, or abdomen, hips, inner and outer thighs. Patients with firm, elastic skin will have a better result.

FAQ Related to Liposuction

Q. I don't like my big belly. Do I need tummy tuck or liposuction?
A. If you have already tried diet and exercise without much success, and you have good skin elasticity and no hanging skin and in good health you are the right candidate for liposuction . If you have hanging skin and poor skin elasticity and loose muscles tummy tuck is the right procedure for you.

Q. Do I need to loose weight before having liposuction?
A. Liposuction is not a weight loss procedure. The best candidates for liposuction are women or men of normal weight that have stubborn localized fat on specific areas of their body such as thighs, abdomen, hips, arms chin and neck that they cannot get rid of with diet and exercise.

Q. When can I go back to work after my liposuction procedure?
A. One week after your procedure you should be able to go to work.

Q. How long will my liposuction surgery will take?
A. The length of surgery depends on the size and number of areas involved. In our Jacksonville Cosmetic Surgery Clinic, Dr. Nezami typically spends 2 hours to four hours for liposuction.

Q. What technique of liposuction should I ask my plastic surgeon to use?
A. During your consultation at our Jacksonville Cosmetic Surgery Clinic, Dr. Nezami will explain different techniques; he will advise you which technique is best for you. Dr. Nezami prefers a combination of the traditional and tumescent liposuction because he can better monitor the amount of fat extracted and remaining fat. Q. Is liposuction procedure permanent? Will the fat cells removed through liposuction comes back?
A. The fat cells that are removed through liposuction procedure will not come back. They are gone forever. However if you gain weight the existing fat cells will become larger, but since you do not have excess fat cell in the area of liposuction, that area become larger proportionate to the rest of the body and not a big bulge.

Q. Do I need to go under general anesthesia for my liposuction procedure?
A. Not necessarily, liposuction can be done under local anesthesia and general anesthesia. Dr. Nezami will decide at your pre-op consultation which method he will use and he will explain the reasoning behind his choice of anesthesia.

Q. Do I need someone to stay with me after my liposuction surgery?
A. Yes, you will need someone to stay with you after your liposuction surgery.

Q. When can I drive after my liposuction surgery?
A. We advise our patients in Jacksonville not to drive before 3 days, and definitely not while they are taking pain medications.

Q. Can I take a shower after my liposuction surgery?
A. You cannot take shower before 3 to 4 days following your liposuction surgery, but you may sponge bath or use hand held shower. You may not take bath for 2 weeks or swim for 3 weeks.

Q. Should I stop taking my medications before my liposuction surgery?
A. We advise our patients in Jacksonville, Jacksonville Beach and Ponte Vedra Beach to avoid taking certain medication such as aspirin, Ibuprofen, Vitamin E, and any other medication that can affect the blood clotting system for two weeks before and after surgery (If you are taking any blood thinner, do not stop until you get authorization from the doctor who prescribed it). Q. How soon can I travel after my liposuction surgery?
A. You can travel 7-10 days after your liposuction surgery.

Q : Do I have to wear a garment?
A. Yes, we use a compression garment immediately after surgery with a special 1 inch thick foam placed over the areas where liposuction was done to prevent bleeding and also to mold the remaining tissue to your body. Then after 10 days we use a second stage garment without the foam and recommend to wear it for 6 weeks. Dr Nezamis believe this is very important to obtain a very smooth and even result from your liposuction. There is variation between plastic surgeons regarding the garment and the amount of time to wear it.
