The enlarged tissue can consist of fatty tissue, glandular breast tissue or a combination. The gland is usually centered under the areola and is attached to the nipple.

Causes of Gynecomastia:

Gynecomastia can occurs when there are some hormonal changes either during infancy, adolescence or old age or infancy when there is a decreased amount of testosterone compared with estrogen. Gynecomastia can also be triggered by a condition that blocks the effect or reduces testosterone or increases estrogen. Several medications can cause gynecomastia such as: androgens, anabolic steroids, Valium, Tricyclic antidepressants, some antibiotics, Cimetidine or digoxin. Alcohol and some street drugs like Amphetamines, Marijuana, Heroin and Methadone can also be the cause of gynecomastia. Hypogonadism, Hyperthyroidism, kidney failure, liver failure, and malnutrition can also create a hormonal imbalance and gynecomastia. Hormonal changes that occur during normal aging in overweight men can also be the cause of gynecomastia. A complete check up with an internist or endocrinologist is recommended to determine the cause of gynecomastia. In majority of cases the cause remains unknown (idiopathic) or is just due to excess fat content in the breast. If it caused by a medical problem, this should be corrected first.

Surgery in general does not stop further growth of the breast. Dr Nezami prefers to operate on stable gynecomastia or those due to excess fat. Gynecomastia surgery can correct over-developed male breast tissue, creating a flatter, firmer and more masculine chest contour. There are two techniques for gynecomastia depending of the amount of excess tissue and skin. Male breast reduction surgery removes the excess tissue by direct excision or with liposuction. Where fatty tissue is the primary source of over-development and there is no excess skin, liposuction techniques remove fat to recontour the chest. For liposuction of male breast, the incision is very small and hidden in the crease of the armpit and under the breast or at the edge of the areola. If there is excess skin and glandular tissue and excess fat excision of the tissue is required. The incision is around the areola and under breast crease line to allow removal of excess skin and glandular tissue. Can also include areola reduction.

Preparing for your Male Breast Reduction Surgery

We advise our patients in Jacksonville and Jacksonville Beach to avoid taking certain medications that thin the blood, such as aspirin and Ibuprofen and vitamin E. If they are smokers they should stop smoking at least three weeks before the surgery. Smoking interferes with blood flow to the skin and prevents the rapid healing and will cause complications. At your pre-operation consultation you will be given specific and detailed instructions.

Incision for Male Breast Reduction Surgery

If liposuction alone is performed, the incisions are very small and are located underarm or in the areola or under the breast. If excision of tissue is needed, the incisions are around the areola and/or on the crease line under the breast. This technique may be performed in conjunction with liposuction to recontour other areas.

Recovery period after Male Breast Reduction

Discomfort is well controlled by oral medication. A compression vest is recommended around the clock for the first week. You should engage in light walking as soon as possible to minimize the potential for blood clots and other complications. A return to light activity is possible as soon as you feel ready, usually within a few days of surgery. You may be able to return to work and more normal activity within a few days, as long as you do not engage in any heavy lifting or vigorous exercise. Initial swelling dissipates in 2-4 weeks. Final results appear in 3-6 months. Incision lines continue to fade for 1 year. Keep in mind that recovery period varies from patient to patient and significant weight gain can reverse results.

Q. What causes gynecomastia ?
A. In addition to weight gain, in adult male, gynecomastia might occur as a result of certain medications such as estrogen derivatives. Steroid taken for bodybuilding may also contribute to gynecomastia. More detail explanation is given above.

Q. When can I go back to work after my gynecomastia surgery?
A. If you work in an office, you can go back to work in a few days after your gyecomastia surgery. If your work involves heavy lifting or is strenuous you should wait two to three weeks.

Q. When can I start exercising?
A. You should wait 4 weeks after your gynecomastia surgery.

Q. Does insurance pay for my male breast reduction?
A. No, because the insurance companies consider male breast reduction as a cosmetic surgery.

Q. For male breast reduction what type of anesthesia will I have?
A. You will have general anesthesia. In our Jacksonville Cosmetic Surgery Center, a board certified anesthesiologist provides anesthesia. The surgery can also be done under sedation and local anesthesia, but for your safety we will still use anesthesiologist.

Q. Do I need someone to stay with me after my gynecomastia surgery?

Q. When can I drive after my male breast reduction?
A. We advise our patients in Jacksonville not to drive before 5 to 7 days, especially if they are taking their pain medications.

Q. Can I take a shower after my male breast reduction surgery?
A. You cannot take shower before 3 to 4 days following your male breast reduction surgery, but you may sponge bathe or use hand held shower to wash below your breasts. You may not take a bath for 2 weeks or swim for 3 weeks.

Q. Should I stop taking my medications before my male breast reduction surgery?
A. We advise our patients in Jacksonville, Jacksonville Beach and Ponte Vedra Beach to avoid taking certain medication such as aspirin, Ibuprofen, Vitamin E, and any other medication that can affect the blood clotting system for two weeks before and after surgery (If you are taking any blood thinner, do not stop until you get authorization from the doctor who prescribed it). Refer to the list of other medication to avoid that will be given to you during your pre-op consultation. Do not stop your prescribed medication until you check with your doctor.

Q. How soon can I travel after my breast reduction surgery?
A. You should not travel for 10 days after your gynecomastia surgery.

Q. I am a smoker; how long before my breast reduction should I stop smoking?
A. For breast reduction you should stop smoking at least 2 weeks before and after the surgery. Smoking interferes with blood flow to the skin and tissue and prevents rapid healing and can cause anesthesia and breathing problems. But if you are having excision of breast tissue in addition to liposuction, then you have to stop smoking for a minimum of one month before and after your gynecomastia surgery. You will need a good blood flow to the skin and nipple for proper healing. Smoking may cause delayed wound healing or even tissue or nipple loss.

Q. When can I lift heavy object after my breast reduction?
A. Three weeks after your male breast reduction surgery you can lift heavy object and there is no more restrictions.

Q. When can I start exercising after my gynecomastia surgery?
A. Three weeks after your breast reduction surgery you may resume exercising, but start slow and progress as tolerated.

Q. Anything special to wear after surgery?
A. Yes, After surgery you will have some minimal dressing on your chest, but you will be placed in a compression vest with hooks in front. Under the vest you will have 2 large pieces of foam just over the areas of liposuction. The after 10 days you can change to a pull down tight undershirt type garment that we recommend to wear for six weeks to get the smoothest possible chest.

Q. Is it going to look like I had surgery on my chest?
A. After everything has healed well and the scar has matured ,in the case of liposuction, there is almost no visible scar. In the case of excision, there will be a scar around the areola and possibly one at the breast crease line, which will not be too visible if you have a hairy chest.
